Pay the $150.00 application processing fee by check (payable to

REACH School) 

3.

Mail the entire application packet to:

Admissions application packet will include the following: 

• Administrator Recommendations Form

• Teacher Recommendations Form

• Signed Release of Records Form

• Current Psycho-Educational evaluation 

• School Transcripts

• Current photo of the applicant 

• Current IEP/504 and Goals
